Our program, "A Community that Cares," initially focused on providing in-home health care to individuals with functional limitations. However, in 2017, we recognized a pressing need for those living without access to essential cleaning and hygiene supplies. Consequently, we shifted our focus to supplementing health and cleaning supplies, expanding our reach from 900 individuals in the first year to over 10,000 cumulatively.
We advocate for individuals in Outagamie County and collaborate with 30 Home Community Based Services (HCBS) organizations. These partnerships address the needs of vulnerable community members, including Medicaid beneficiaries, Children’s Long-Term Support recipients, and those enrolled in programs like Family Care and IRIS.
Compassionate Home Health Care's Health and Hygiene Popup Hub aims to support families and early childcare professionals. By promoting parenting skills and providing access to supportive family services, we strive to prevent health and hygiene poverty. Additionally, we offer medical equipment donated by our partner organizations on a first-come, first-served basis, further enhancing our impact on community health and well-being.
